The front of this log book includes two pages for recording signatures and capturing the initials of users. The following 62 pages are controlled substances use logs. Each log includes prompts at the top to record the name of the Controlled Substance, Class, Concentration, Form, Bottle #, Serial #, Expiration Date, and Size of Container. Beneath, there are 14 rows for documenting the usage of that bottle, including prompts for the Date, Initials of User, Witness Initials, Amount Used, Amount Wasted, Amount Added, and Ending Volume. A notes section follows for recording discrepancies. Following the 62 log pages that follow this format, there are 18 additional lined pages for further notes. Each logbook should be kept for 2 years.
