We've seen all kinds of different coffee-to-water ratios, but we like to keep ours simple: 2 ounces of coarse-ground coffee to one quart (4 cups) of water and a long 24-hour steep. Dilute the concentrate with water, ice, and/or your favorite dairy/non-dairy mixers. As with all things coffee-related, adjust to your taste. Store cold brew in the fridge for up to a week - but it's so good it won't last nearly that long!
