The contents could have included some details of pertinent anatomy and biomechanics, but this background material is well known to physiotherapists and there are ever so many texts available. My most valuable self-learning and teaching tools have been my articulated plastic spine and the articulated bones of the upper and lower extremities. These being flexible and accurate in detail, makes it simple for me to show and explain the patients what I propose to do. Brian Mulligan’s concept utilizes mobilizations with movement (MWMS) in the extremities and sustained natural apophyseal glides (SNAGS) in the spine to relieve pain and increase range of motion. All the mobilisations I use are always applied parallel to the treatment planes of the facet joints. I may also use distractions at right angles to the treatment plane. This complies with the rule that applies for extremity joint mobilisations as described in Freddy M Kaltenborn's book Mobilisations of the Extremity joints. Here he describes the treatment plane as lying across the concave articular surface. The treatment plane moves with the concave partner. Physiotherapists must be famililar with the direction of all spinal facet planes.
